We are happy to offer you the ability to fill your forms out online. You will enjoy the convenience of not having to print anything out! If you don’t have access to a computer, don’t worry – we have iPads in the office. Just come a few minutes before your appointment and do it then. It’s just another way Oregon City Dentistry is trying to go paperless. We want to do our part!

Dental Insurance
We are happy to bill all insurance plans. However, please check with your insurance carrier to see if you are on a preferred provider plan. We are a preferred provider for Delta Dental Premier. Our Insurance Specialist will assist you in assessing your benefits and billing your provider. Co-payments are due at the time of treatment.
